No, the medical exam form is there for convenience. If your provider has their own form they prefer to fill out, or if they prefer to give you a visit summary, that is acceptable--as long as your name and DOB are included on the original document--and can be uploaded to Med+Proctor.
Consult your healthcare provider for guidance. If your provider recommends you do not receive the vaccine, please have them complete a medical exemption waiver request form. Please upload the form to Med+Proctor and email the Wellness Center after doing so.
Student Health Insurance Plan
All domestic undergraduate students are required to show evidence of an existing, creditable health insurance policy. Otherwise, they will be enrolled automatically in the Student Health Insurance Plan (SHIP).
The Student Health Insurance Plan (SHIP), provided by UnitedHealthcare and sponsored by the college, includes the following:
- Individual deductible per policy year:
- $250 in-network
- $600 out of network
- Pharmacy benefits
- $25,000 accidental death benefit
- Academic Emergency Services Global Emergency Package (including repatriation, medical evacuation.
Between 6/20/2024 and 7/31/2024, please visit the waiver website, and follow the steps to accept coverage.

No; with the college-sponsored student health insurance plan, only currently enrolled undergraduate students are eligible to enroll.
When you graduate, you’ll still be covered by the college-sponsored student health insurance plan until your plan expires, as long as your premium has been paid. Fall plan coverage is from 08/01/24 to 12/31/24. Spring/summer plan coverage is from 01/01/25 to 07/31/25.

Yes. The college-sponsored student health insurance plan provides coverage in all 50 states, U.S. territories and foreign countries. Foreign nationals are not covered in their home country.
No. You don't need a referral from the Wellness Center with SHIP. If you visit an off-campus medical provider, you'll be responsible for co-pays, deductibles and any portion of the bill not covered by SHIP. You should verify with United HealthCare before scheduling an appointment to ensure that the provider is covered by United Healthcare’s plan.
If you're already covered by an insurance policy (through parent/guardian’s plan, employer’s, or partner’s plans), you can submit your insurance policy information for verification completely online through Academic HealthCare’s website during the waive period between 6/10/2024 and 7/31/2024.
Your plan should provide benefits similar to SHIP's plan; it must be ADA compliant and allow you to receive services within 50 miles of Decatur/Atlanta, GA.
Hard waiver” does not mean that you must enroll in the college-sponsored student health insurance plan (SHIP). It means that you must show evidence of adequate coverage by an acceptable health insurance plan.
Being enrolled in SHIP is one means to meet student health insurance requirements. We encourage you to compare SHIP against other options (for example, being a dependent on a parent/guardian’s health insurance plan, enrolling in a partner’s health insurance plan, or enrolling in an employer’s health insurance plan).

Yes, all registered students may seek health services and counseling from the Wellness Center.
Hard waivers are monitored and verified by Academic HealthPlans. Academic HealthPlans will send an email message to you regarding the acceptance, need for further information, or denial of your waiver application.
No. You only need to complete the waiver process once each academic year, prior to the fall semester.
